Kaiser Diabetic Shoe Program

If you want to take the first step towards healthier feet, make an appointment with your doctor at Kaiser Permanente. Your doctor will refer you to a podiatrist who will evaluate your feet and determine what therapeutic option is best for you.

Your Kaiser Rx will be faxed to us. Once we receive it, we will call you to schedule an appointment at our office with one of our pedorthists. Please note that it may take 1—2 weeks before we receive your Rx from Kaiser.

During your appointment, you and your pedorthist will discuss your particular situation and the proper solution. If your Rx prescribes custom shoes or custom orthotics, we will take molds (casts) of your feet at this time.

As soon as your order is complete, we will call you to set up a dispense appointment.

Depending upon your coverage, a co-pay may be required on your first visit.

 

STEP 1:
Your Kaiser Doctor determines you need pedorthic care and submits a Rx for authorization.

STEP 2:
Once we receive your authorized Rx from Kaiser, we contact you for an appointment.

STEP 3:
During your appointment, which lasts approximately an hour, we assess your needs and determine what is needed to fill your footwear Rx.

STEP 4:
When your order is complete, we notify you to schedule a dispense appointment.

STEP 5:
Enjoy your new-found comfort!
